i think it's the first time i've seen them in the yard

#fail i believe originated with twitter:
when the site is overloaded
you get a splash page w/a whale on it:

the # sign is used on twitter to identify a word that should function as a tag

so if you tweet:
does anyone know where the #fire is in brooklyn that is making that awfull smell
it will make your tweet come up more easily if someone searches on "fire"

#fail has become a somewhat ironic way of saying something f*cked up
